There's been a big misunderstanding in the medical SLP space...

You've probably heard someone say:

"NMES doesn't work."

Or...

"Research shows it isn't any better than traditional dysphagia therapy."

It's true that there are published studies reporting findings like these.

Two of the most frequently cited are the studies by Langmore et al. and Humbert et al.

However, it's important to ask...

Which NMES protocol did they actually study? Because it wasn't Ampcare's Effective Swallowing Protocol (ESP).

Before we take the results of any NMES study and apply them to every form of NMES, we should ask:
➡️ Which NMES device was used?
➡️ Where were the electrodes placed?
➡️ What stimulation parameters were used?
➡️ Was the stimulation sensory-level or motor-level?
➡️ What swallowing exercises were performed during stimulation?
➡️ What patient population was studied?

"NMES" isn't one standardized treatment. Different devices, electrode placements, stimulation parameters, exercise protocols, and patient populations can all influence outcomes.

That's why evidence-based practice means looking beyond a study's conclusion and asking what intervention was actually tested before assuming the findings apply to every NMES protocol.

The takeaway: research findings should be interpreted in the context of the specific NMES protocols they evaluated, not automatically generalized to all forms of NMES!

All he wanted was a coney chili cheese dog, but instead he was stuck with a PEG tube. Until he met Shannon....

He had a history of throat cancer treated with chemo and radiation and had been cleared since 2013.

After worsening dysphagia, he became PEG-dependent and started therapy NPO.

After just 9 weeks of Ampcare ESP, he progressed from NPO + PEG to thin liquids and a regular diet.

He told Shannon he was getting a 12-inch chili cheese coney as his first meal.

Shannon wisely suggested starting with a 6-inch 😅

And he enjoyed every. single. bite.

📣Quick reminder for SLPs: active cancer is different from a history of cancer. 📣

With the right medical history, clearance, and clinical reasoning, head and neck cancer survivors may be appropriate candidates for Ampcare.

As long as the cancer is not ACTIVE, then Ampcare is not contraindicated.

We haven't met many SLPs yet who learned about this specific exercise in grad school - but it's critical!

This jaw movement has almost nothing to do with the jaw itself. Almost.

During Ampcare's Effective Swallowing Protocol (ESP), resisted jaw opening and closing is used to challenge the suprahyoid muscles (the anterior digastric, mylohyoid, and geniohyoid) across their origin and insertion.

i.e., we exercise these muscles across BOTH points of attachment (the hyoid and the mandible).

When the jaw is stabilized, the suprahyoids move the hyoid UP and FORWARD (aka a swallow)

When the suprahyoids are stabilized (by the infrahyoids), the suprahyoids will tug the jaw open.

Why does that matter? These muscles play a major role in hyolaryngeal excursion during swallowing.

By resisting jaw opening and closing with the posture device, we're targeting the muscles responsible for generating a stronger contraction where it counts.

Think of it like a resistance band attached between two moving points. Depending on which end is stabilized, the direction of the work changes, but the band is still under tension.

So, while the jaw movement is easy to see...

The real work is happening underneath it.

All three muscles: genioglossus, styloglossus, and hyoglossus, are innervated by the HYPOGLOSSAL nerve.

These muscles and nerve are simply too deep for transcutaneous NMES to reach effectively.

🙋But wait, if we can't stimulate these muscles directly, how does Ampcare's ESP create tongue movement?

The secret is anatomical inference, a brilliant chain reaction!

When ESP stimulates the suprahyoid muscles (anterior digastric, mylohyoid)....
..it moves the hyoid bone.

⚡🦴👅

The hyoglossus muscle attaches to the hyoid. When the hyoid moves up and forward, it pulls the tongue along with it.

Think of it like this:
We're not trying to control the tongue directly. We're moving the foundation it's attached to.

The kinematic movement is the key: it's the hyoid attachment that moves the tongue, not direct nerve stimulation.

🙌This is why ESP works WITH your body's natural connections instead of trying to force stimulation where it can't effectively reach.
