We give every client a book we've spent 20 years building.

It's not a care plan. Not a task checklist.

It's a collection of questions we use to keep our clients engaged, talking, and connected to their own story. The caregiver isn't on their phone. The client isn't on hour four of Judge Judy. They're having a real conversation.

We use it with our dementia clients too. The answer doesn't have to be correct — the engagement is the point. And what comes out of those conversations surprises everyone, including the family.

Because the answers become a memoir. Personal details. Stories. Opinions. Things their own children never knew. Written down. Preserved.

What our caregivers in Hamilton say after a year on the team.

Spring to summer in Manalapan: three medication categories worth asking the doctor about before the heat hits.

Older adults are more sensitive to heat than they were at 50. Several common medications make that sensitivity worse.

Three categories worth a conversation with the prescribing doctor in May:

1. Diuretics (water pills). Common for blood pressure and heart conditions. They increase dehydration risk in summer heat.

2. Beta blockers. Common for blood pressure. They can blunt the body's normal heat-regulation response, so seniors do not feel as hot as they actually are.

3. Antihypertensives in general. Many lower blood pressure, which combined with summer dehydration can cause dizziness, fall risk, and fatigue.

Not a reason to stop any medication. A reason to ask the doctor at the next visit: 'Is anything Mom is taking heat-sensitive?' 'What signs of dehydration should I watch for?' 'Should we adjust timing for summer?'

A 10-minute conversation now prevents a 911 call in July.
