The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient's needs. The home health aide is responsible for observing patients, reporting these observations, and documenting observations and care performed.

The home health aide will be assigned in a manner that promotes the quality, continuity, and safety of a patient's care.

Essential job functions/responsibilities

Responsibilities of the home health aide include, but are not limited to, the following:

1) Providing hands-on personal care including:

A. Baths
B. Back rubs
C. Oral hygiene
D. Shampoos
E. Changing bed linen
F. Assisting patients with dressing and un******ng
G. Skin care to prevent breakdown
H. Assisting the patient with toileting activities
I. Keeping patient's living area clean and orderly, as appropriate

2) Planning and preparing nutritious meals.

3) Assisting in feeding the patient, if necessary.

4) Taking and recording oral, re**al, and auxiliary temperatures, pulse, respiration, and blood pressure when ordered (with appropriate completed/demonstrated skills competency).

5) Assisting in ambulation and exercise according to the plan of care.

6) Performing range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)/

7) Performing simple procedures as an extension of nursing services as ordered (with appropriate completed/demonstrated skills competency).

8) Assisting patients in the self-administration of medication.

9) Doing patient's laundry, as appropriate.

10) Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc).

11) Reporting on patient's condition and significant changes to the assigned nurse.

12) Participates as a member of the interdisciplinary team.

13) Adhering to the organization's documentation and care procedures and standards of personal and professional conduct.

Position qualifications

1) Meet the training requirements and successfully completes:

A. A training and competency evaluation program that meets §484.80 (b) and (c); or

B. A competency evaluation that meets the requirements of §484.80 (c); or

C. A nurse aide training and competency evaluation approved by the state as meeting the requirements of §483.151 through §483.154 and is currently in good standing on the state nurse aide registry; or

D. The requirements of a state licensure program that meets the provisions of §484.80 (b) and (c).

2) Shows proof of registration on the Home Care Aide Registry through the Department of Social Services, Home Care Services Bureau, preferred.

3) At least 18 years of age.

4) Ability to read and follow written instructions and document care given.

5) Self-directing with the ability to work with little direct supervision

6) Empathy for the needs of the ill, injured, frail, and impaired.

7) Possess and maintains current CPR certification

8) Demonstrates tact, patience, and good personal hygiene.

Job limitations

The home health aide will not function in any manner viewed as the practice of nursing according to the State's Nurse Practice Act. Specifically, the home health aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge, and skill of a nurse, such as sterile techniques.

The medical social worker is responsible for the implementation of standards of care for medical social work services.

Essential job functions/responsibilities

1) assesses the psychosocial status of patients related to the patient's illness and environment and communicates findings to the registered nurse.

2) Carries out social evaluations and plans intervention based on evaluation findings.

The speech-language pathologist is responsible for the implementation of standards of care for speech-language pathology services and for adherence to all conditions in the Service Agreement.

Essential job functions/responsibilities

1) Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.

2) Identifies functional speech defects and establishes a plan of care to improve the patient's function.

3) Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.

4) Recommends communication devices/aids as indicated

5) Confers with referring physicians and other organization personnel to ensure coordinated and comprehensive care contributes to the total plan of care.

6) As appropriate, participates in case conferences, in-services, and other meetings required to ensure coordinated and comprehensive care.

8) Maintains clinical competency in speech-language pathology practice and theory.

9) Identifies patient and family/caregiver needs for other services and refers as appropriate.

1) Evaluates patient's functional status (muscles function, endurance, visual coordination, written and verbal communication skills, self-care ability, work capacity, etc.) Evaluates home environment for hazards or barriers to more independent living. Identifies equipment needs. Participates in the development of the total plan of care.

2) For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with organization policy.

3) Develops treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other organization professionals.

4) May teach new skills or retrain patients in once familiar daily activities that have been lost due to illness or injury, in accordance with organization policy.

5) May fabricate splints and instruct patients in the use of various types of adaptive equipment to improve function.

6) May train patient in the use of prosthetic and/or orthotic devices.

7) Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in the patient's condition. Submits these records in accordance with organization policy.

8) Maintains contact/communication with other personnel involved in the patient's care to promote coordinated, efficient care.

9) Attends and contributes to in-services, case conferences, and other meetings as required by organization policy to ensure coordinated and comprehensive plans of care for the patients of the organization.

8) Provides physical therapy consultation to home families and caregivers when indicated.

9) Provides in-service education programs for nursing organization personnel as needed.

10) Participates in peer consultation processes.

11) Supervises Physical Therapy Assistants according to organization policy and state regulations.

12) When therapy is the only skilled service, instructs, supervises, and evaluates home health aide care every two weeks
