Starting August 1, 2025, EmblemHealth will require preauthorization for certain surgeries performed in hospital outpatient settings (places of service 19 & 22) for patients under age 75.

-Surgeries in hospitals may not be approved unless deemed medically necessary.
-The same surgeries performed in an Ambulatory Surgery Center (ASC) like Queens Endoscopy will not require preauthorization.

This means:

✔️ Lower out-of-pocket costs
