Elevated Community Health We proudly offer healthcare services to the medically underserved, uninsured, or underinsured at affordable costs.

Elevated Community Health is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental, and behavioral health services through a team-based model of care. Summit Community Care Clinic is a Federally Qualified Health Center (FQHC) that provides a full range of integrated care services inclusive of medical, dental and behavioral health services through a team-based model of care. We also accept Medicaid, Medicare and private insurance. The Care Clinic works with patients to make sure their health needs are met first and foremost. The Care Clinic knows that healthy people are able to stay in work, provide for their families, and contribute to our local community. Service fees for all patients without insurance are based on income and family size. Patients without insurance will be charged according to a sliding fee scale—based on Federal Poverty Guidelines—if deemed income eligible. No patient will be denied service for their inability to pay.

Looking for a simpler way to plan for your healthcare costs? Elevated Essentials is designed to help make care more accessible for uninsured individuals and families, as well as businesses looking for healthcare options for their employees.

Choose the services that fit your needs, including primary care, dental, and behavioral health, with clear, prepaid plan options designed to make costs easier to understand.

Our Care Card program and Medicaid are still here. Elevated Essentials does not replace these programs. Our eligibility team will continue to help patients explore their options and ensure those who qualify are enrolled in Medicaid or our sliding fee discount program, the Care Card.

Our goal is to help every patient find the most appropriate and affordable care option available to them.

Your voice matters in shaping care for our community. Save the date for Elevated Community Health’s Patient Advisory Council on September 15 from 5:30-7:00pm. It’s an opportunity for patients to share feedback, ideas, and perspectives that can help us continue improving the care we provide. This session will be bilingual with a live translator.
