It may sound unusual, but this noticeable change in the appearance of the upper arm can be a sign of a proximal biceps tendon rupture.

When we talk about “quad tendon ruptures,” most people picture a complete tear at the top of the kneecap — usually from a forceful eccentric load like a heavy landing or a rapid deceleration. But this one is different.

This is a re**us femoris rupture at the musculotendinous junction — the zone where muscle fibers hand off to tendon. It’s a region built for force transfer, and it’s a classic failure point during explosive activity: sprinting, kicking, a sudden eccentric load when the muscle is firing hard while being stretched.

Why it matters:

▪️The re**us femoris is the only quad muscle that crosses both the hip and the knee, so it lives under high strain.
▪️Injuries here behave differently than a distal quad tendon tear at the patella — the presentation, the imaging, and the treatment conversation all shift.
▪️Many of these are managed without surgery, but the decision depends on the degree of retraction, the athlete’s demands, and how the tissue is healing — which is exactly why accurate diagnosis and imaging matter.

This summer, we had the pleasure of welcoming Michelle as an intern with Dr. Dold at Star Orthopedics. Michelle is a rising senior at Harvard University studying biomedical engineering with a secondary in chemistry. Her experience working with Dr. Dold has strengthened her interest in orthopedics, and she plans to attend medical school after taking a gap year post-graduation.
