Stop telling me testosterone is only undertreated in women.

Because men aren’t getting great care either.

We often talk about how women are undertested, undertreated, and underserved when it comes to testosterone. And that’s true. The consequences—low libido, fatigue, brain fog, loss of muscle, depression, and declining quality of life—are real.

But a new study asked a simple question:

Are doctors even following the testosterone guidelines for men?

The answer?

Not really.

Researchers looked at more than 132 million patients across 72 healthcare organizations. The American Urological Association recommends checking testosterone in men with conditions like anemia, type 2 diabetes, osteoporosis, infertility, and more.

What actually happened?

➡️ Men with anemia: 1.4% were tested.
➡️ Men with type 2 diabetes: ~6% were tested.
➡️ Even after a low testosterone level was found, many never received the recommended confirmatory testing.
➡️ And even after confirmation? Treatment rates remained shockingly low.

The one exception?

Male infertility. More than 90% of men with confirmed testosterone deficiency were treated.

Why?

Because specialists were involved and fertility was the priority.

Funny how care improves when the right expert is in the room.

This isn’t just a women’s issue.
It isn’t just a men’s issue.

It’s a medical education issue.

Guidelines exist. Evidence exists. Patients exist.

But if we don’t test, we can’t diagnose.
And if we don’t diagnose, people continue suffering from treatable conditions.

Women make up nearly 2/3 of Alzheimer’s cases.

And a brand new genetics paper is raising a very important question:

What if testosterone matters for women’s brain health too?

Researchers looked at genetic data from nearly 1 million people and found that higher bioavailable testosterone in women was associated with a 12% lower risk of Alzheimer’s disease.

Some considerations here:
➡️ This is a preprint, not yet peer-reviewed.
➡️ This does NOT prove testosterone prevents Alzheimer’s.
➡️ But it does add to the growing pile of evidence that women’s hormones include testosterone and are brain hormones.

One of the most interesting parts?
The researchers found stronger Alzheimer’s-related genetic signals connected to testosterone biology in women than in men.

We have spent decades talking about estrogen and the female brain. Meanwhile testosterone in women has been ignored, minimized, stigmatized, or treated like it only matters for libido.

But testosterone affects:
• brain function
• motivation
• energy
• muscle
• metabolism
• inflammation
• and likely cognitive aging

Women deserve inclusion in the testosterone conversation. Full stop.

And this is why I keep talking about hormones as MORE than symptom management. This is about long-term health, aging, function, and quality of life.

In a world where women have way more dementia than men…..don’t forget to research the testosterone.
