CCACC Health Center 泛亞門診

CCACC Health provides primary care, preventive care, health-related services, and welfare applications for uninsured low-income residents of Montgomery County.

Community health clinic, serving Montgomery County, MD since 2003. Services:
- Primary Care
Diagnosis and treatment of acute and chronic illnesses (i.e. diabetes, hypertension, and asthma).
- Referral Services:
Patient referrals to connected facilities for diagnostic imaging at a discounted rate. Patient referrals to specialists and health management plans for follow-up care.

Requirements:
- Montgomery County resident
- Uninsured
- Income below 250% Federal poverty level
- Age 18+

Bilingual Staff (Chinese).

Asian Americans were the least likely to receive counseling or therapy from a mental health professional in 2024 (CDC).

Across the U.S., 14% of adults received counseling or therapy, but for Asian Americans, that number drops to 8.1%.

Why the gap?

We are pleased to cordially invite everyone to CCACC Health Center's Annual Health Fair 2026. This annual fair will bring together our community members, including local organizations and volunteers, and healthcare providers for a day dedicated to promoting one's health and wellness and access to medical care.

We would be honored to have everyone use this day of togetherness to get resources on how to maintain one's health as well as learn more about the importance of preventive screening.
