Anterior cruciate ligament ( ) rupture is a traumatic knee injury affecting approximately 200 000 people annually in the US.

Clinical guidelines vary, but high-quality evidence supports starting with structured rehabilitation in most adults, with ACL reconstruction reserved for persistent instability or high activity demands.

No significant differences in 5- to 10-year osteoarthritis rates or return to sport are observed between early surgery and initial rehabilitation, and surgical graft options—including hamstring, bone-patellar tendon-bone, and quadriceps tendon—produce comparable failure rates and complications.

Prehabilitation before surgery improves postoperative outcomes, while return to sport is associated with a notable risk for second ACL injury.
