Community HealthNet Health Centers CHN provides comprehensive Family, Pediatrics, Behavioral Health, Eye Care, Dental, OB, and Prenatal Medical Services. Parking Lots available.

Our OB and prenatal services are equipped with technology to conduct onsite ultrasounds. Community HealthNet (CHN) Health Centers is a not-for-profit community based primary healthcare organization dedicated to providing accessible, affordable, and quality medical services. Community HealthNet strives to serve the needs of all families and individuals of all income levels in need of medical care.

To achieve this commitment, CHN will work in collaboration with other health care providers and service-oriented agencies in the community to provide a continuum of care. In addition, CHN has partnered with Covering Kids & Familes (CKF) to assist individuals who qualify to enroll in a health insurance plan. Hours of Operation CHN

Community HealthNet Health Centers is helping families get ready for a healthy and successful school year! Now is the perfect time to schedule your child’s school physical and required immunizations before classes begin.

School vaccines play an important role in protecting students, families, and our community from preventable diseases. Staying up to date on immunizations helps keep children healthy, reduces the spread of illness, and ensures students meet school enrollment requirements.

We were honored to participate in Lake Ridge Elementary Head Start event on Friday, June 5, 2026, from 10:00 AM – 1:00 PM, where we connected with families and shared important health resources to support student wellness.
