Join our Team! PhyNet is seeking an LVN to support the care team in a office setting focusing on remote coordination of care for patients with chronic conditions. This role emphasizes follow-up, care plan support, and maintaining continuity across care settings.
CORE DUTIES INCLUDE:

• Conduct outreach to patients for chronic care follow-up, medication reviews, and health assessments.
• Coordinate referrals, diagnostics, and provider communication across multiple care sites.
• Update and maintain accurate records in the EHR.
• Experience in chronic care management or care coordination preferred.
