The first few weeks after a stroke are a critical part of recovery—not simply a waiting period between hospital discharge and the next appointment.
During this time, patients may need medication adjustments, blood-pressure monitoring, review of test results, rehabilitation planning, and evaluation for new or worsening symptoms. The risk of another vascular event is also highest early after a stroke, which makes timely follow-up especially important.
Research has also shown that roughly 1 in 8 patients may be readmitted within 30 days after an ischemic stroke, often because of recurrent vascular problems, infections, medication issues, or other complications. Early outpatient follow-up can help identify these concerns sooner and support a safer transition from hospital to home.
At Bare Face RVA, we are expanding post-stroke care with a focus on:
• Reviewing the cause of the stroke and pending hospital studies
• Optimizing blood pressure, cholesterol, and stroke-prevention medications
• Addressing mobility, speech, fatigue, cognition, mood, and return-to-work concerns
• Coordinating rehabilitation and specialty follow-up
• Helping patients and families understand what comes next
The goal is not only to survive a stroke—but to recover well, reduce the risk of another stroke, and feel supported during one of the most important stages of care.
