I recently had someone come into the office because they were struggling with fatigue.

As we talked through everything they were doing, they told me they were taking Armour Thyroid, additional T3, and several mitochondrial supplements because they had been told those things would help increase their energy.

So I asked them a simple question.
"If all of those things are supposed to increase your energy, why are you still so tired?"

They told me they actually did feel better in the beginning. The extra T3 seemed to help for a while. Some of the supplements gave them a boost, too. But eventually the improvements stopped lasting. They were increasing doses, adding more supplements, still crashing during the day, and now they were having trouble sleeping.

This is something I see all the time.
When someone has low energy, we're often taught to ask, "What do I need to take?" More thyroid hormone. More mitochondrial support. More nutrients.

But I think we're asking the wrong question. 💡
Why is the body reducing energy production in the first place?

That question has shaped the way I practice for decades, and it's one of the central ideas behind my new book, Is This the Hill?

One of the biggest misconceptions I see is that if you have low energy or "mitochondrial dysfunction," the answer must be more support.

More supplements.
More thyroid hormone.
More strategies to increase energy production.

Sometimes those things help. Sometimes they don't. And sometimes they help for a while... until they stop.

The better question is: Why is your body reducing energy production in the first place?

This week we've spent a lot of time talking about mitochondrial testing, adaptive physiology, and why context matters.

If there's one thing I hope you take away, it's this:
An abnormal test result is only one piece of the puzzle.

Understanding your symptoms, your physiology, your blood chemistry, and how your body is adapting is what leads to better clinical decisions—not simply finding another abnormal marker to treat.

Too often, people are told they have "mitochondrial dysfunction" and are immediately started on supplements without ever asking why those changes occurred in the first place.

Your body is always responding to the environment it's in. The better we understand those adaptations, the better we can determine what support is actually appropriate.

Mitochondrial testing can provide useful information—but only if the results are interpreted in the right context.

Too often, abnormal mitochondrial markers are treated as evidence that the mitochondria are broken, leading to long supplement protocols aimed at “fixing” them.
The reality is that mitochondrial function is influenced by far more than nutrient status.

Inflammation, immune activation, thyroid hormone signaling, oxygen delivery, toxins, under-fueling, overtraining, sleep, and overall physiologic state all affect how your mitochondria produce energy. In many cases, reduced mitochondrial function may be part of your body’s adaptive response—not the primary problem itself.

In this week’s Thyroid Thursday, I explain what mitochondrial testing can and cannot tell you, why context matters when interpreting the results, and how this fits into a state-based approach to physiology.

An abnormal mitochondrial test result can provide valuable information.

But it's important to understand what that information actually represents.

A test may show reduced Complex I activity, altered organic acids, abnormal acylcarnitines, or other markers associated with mitochondrial function. Those findings describe what is happening at the time the test was performed.

What they don't tell you is why those changes occurred. 🔍

Reduced mitochondrial function may be influenced by inflammation, immune activation, altered thyroid hormone signaling, poor oxygen delivery, under-fueling, toxin exposure, sleep disruption, or other physiologic stressors. In some cases, those changes may even represent an adaptive response rather than permanent dysfunction.

That's why two people can have similar test results but require completely different treatment strategies.

Testing should always be interpreted within the context of the person's history, symptoms, blood chemistry, physiology, and overall health—not in isolation.

If you've been told you have "mitochondrial dysfunction," it's easy to assume you've found the cause of your symptoms.

But an abnormal mitochondrial test doesn't automatically mean your mitochondria are broken.
Mitochondrial testing can reveal how your cells are producing energy, but those findings don't exist in isolation. Inflammation, immune activation, thyroid hormone signaling, oxygen delivery, nutrient availability, toxin exposure, and overall physiologic state can all influence the results.
That's why the same test finding can mean something completely different from one person to the next.

I was asked recently whether someone struggling with chronic symptoms should have mitochondrial testing done.

My answer?
Maybe... but probably not as the first step.

If you've been dealing with fatigue, thyroid issues, brain fog, or other chronic symptoms, there's a good chance your mitochondria aren't functioning optimally. But that doesn't automatically mean your mitochondria are the problem.

Too often, people spend hundreds or even thousands of dollars on specialized testing only to be told they have "mitochondrial dysfunction" and need a long list of supplements.
The problem is that the test can tell you what is happening, but it usually can't tell you why it's happening.

Reduced mitochondrial function may be related to inflammation, immune activation, altered thyroid hormone signaling, poor sleep, under-fueling, infections, toxin exposure, or other stressors that are causing the body to intentionally reduce energy production.

Without understanding why that adaptation occurred, it's easy to spend a lot of money chasing the test result instead of addressing the underlying physiology.
If you're wondering whether your symptoms could be related to reduced thyroid physiology, poor mitochondrial function, or another underlying stress response, start by understanding the factors that may be influencing your body's ability to produce energy.

One of the biggest misconceptions I see is the belief that if we can "optimize" a lab value, we'll automatically restore health.
Maybe it's T3.
Maybe it's cholesterol.
Maybe it's estrogen, testosterone, progesterone, or another hormone.
The number changes...
But the symptoms don't.

This is often why people end up taking multiple medications, hormones, and handfuls of supplements, yet still struggle with fatigue, brain fog, digestive issues, weight changes, and other chronic symptoms.

The problem is that a lab value is rarely the problem.
It's information.
It tells us how your body is responding to its current physiologic state. Simply changing the number doesn't necessarily address why your body adapted in the first place.

If you've been told your labs look great but you still don't feel like yourself, it may be time to take a step back and look at the bigger picture.
