High Ppeak + normal Pplat = resistance problem Think: bronchospasm, secretions, kink, biting, circuit obstruction.

High Ppeak + high Pplat = compliance problem Think: edema, ARDS, atelectasis, pneumothorax, pneumoperitoneum/ Trendelenburg.

Same framework in the ICU and OR.
