This year’s theme, “In Full Bloom: The Future of IVF,” brought together leading clinicians, scientists, embryologists, and industry partners who shared the latest innovations shaping reproductive medicine and fertility care. I had the opportunity to learn from experts and connect with colleagues. I’m excited to bring back new insights to advance patient care and improve outcomes.

The 2026 American Society for Reproductive Medicine recurrent pregnancy loss (RPL) guidelines were updated!!!

Here are the key changes:
RPL is defined as 2 or more losses, including biochemical pregnancies confirmed by hCG and losses that are not consecutive.

Recommended evaluation:
Genetic testing of miscarriage tissue (preferably chromosomal microarray),
Uterine cavity evaluation (saline sonogram, HSG, hysteroscopy),
Psychological support and preconception counseling
Testing considered in selected patients:
Antiphospholipid syndrome (APS) labs
Thyroid testing (TSH)
HbA1c if diabetes risk factors
Parental karyotypes
Chronic endometritis testing
S***m DNA fragmentation testing

Treatments supported:
APS: low-dose aspirin + heparin
Hypothyroidism: treat if TSH >4 mIU/L
Surgical correction of some uterine abnormalities
Metformin may help in PCOS/insulin resistance cases

ASRM specifically advises against routine:
NK cell testing
IVIG, intralipids, prednisone
Broad inherited thrombophilia testing
Aspirin/heparin without APS
Thyroid antibody testing if TSH is normal
ERA or microbiome testing

IVF with PGT-A may be considered selectively, especially with genetic issues or advanced maternal age.

PCOS (polycystic ovarian syndrome) is now being renamed PMOS (Polyendocrine Metabolic Ovarian Syndrome) and honestly, it’s a long overdue shift. For years, the name PCOS focused only on ovaries and cysts, when the condition impacts so much more: hormones, metabolism, mental health, fertility, skin, weight, and overall wellbeing. Many people with PCOS don’t even have ovarian cysts, which led to confusion, delayed diagnoses, and being dismissed.
The new name aims to better reflect the reality of living with this condition and push for more accurate care, research, and understanding.
A name change alone won’t fix everything, but recognition matters. Awareness matters. Being heard matters. To everyone navigating PMOS/PCOS every day: your symptoms are real, your struggles are valid, and you deserve proper support and care!

Happy Doctors’ Day 🩺 I got pretty spoiled today: cake, flowers, breakfast,ice cream bar, the sweetest card… 🤪
But today isn’t just about recognition—it’s about gratitude. I’m incredibly thankful to be part of a team that shows up every day with compassion, resilience, and dedication. Medicine is never a solo journey, and I’m constantly inspired by the nurses, embryologists, PAs, MAs, support staff, and fellow physicians who make it possible to care for our patients the way they deserve.
Thank you for your teamwork, your strength, and the countless unseen moments that make a real difference.

Today was one of those wonderfully busy days at TFC —the kind that reminds me why we do what we do. 8 retrievals, two transfers, one IUI, two new patients and 30+ IVF plans later…in the middle of the pace, pressure, and emotion that come with this work, I’m reminded how grateful I am for our team and patients. The nurses who bring calm to the chaos, the embryologists who work with such precision and care, the front-desk and support staff who keep everything moving with a smile.
It takes every single one of YOU to walk this journey with our patients, and I’m proud of the heart, resilience, and teamwork that show up here every day.
Busy days aren’t easy—but with this team, they’re meaningful. So thankful. Happy Thanksgiving 🦃🍁
