Congratulations, Region 5!
The seven-county Region 5 coalition has begun implementing $16.7 million in Growing Rural Opportunities for Well-Being (GROW) initiative grants, part of the state's largest-ever investment in rural health care. Region 5 includes Clay, Greene, Owen, Parke, Putnam, Sullivan and Vermillion counties, and the award will fund five connected activities that residents across the region will begin to see over the coming year.
This award is the result of seven counties deciding to solve the same problems together. As the coalition moves from planning into implementation, we can begin work in communities where long distances and limited specialty care mean too many people miss the appointments they need. Over the next year the coalition will lay the foundation for initiatives that will identify patients carrying the heaviest burden of coordinating their own care, connect them to services that make a measurable difference, track outcomes, and keep learning how to serve our communities better. The work starts now.
