For a lot of providers, the workday doesn't end when the last session does.
You see your last patient at 5 PM, and on a good day the note gets finished before you leave. On other days it waits until after dinner, gets reopened at 9 PM, and the workday is still going.

Hint: reviewers check whether a session ties back to a goal on the treatment plan, and without that connection, even good care can look like it happened without a clear direction.

The good news is fixing it doesn't mean writing more. Usually it's one sentence naming the goal you were working toward.

Some of what you write down during a session belongs in the note, and some of it is just for you.
