Please see attached the Mobile Health Clinic Calendars for the month of September 2026. The Mobile Health Clinic will continue to practice COVID 19 measures. Patients/Guests will be asked to social distance and wear masks during their visit.

Reminders:
• Due to safety concerns, only the patient will be allowed in clinic area. Minor patients may be accompanied by one parent.
• All patients must present a picture identification that has name and date of birth.
Forms of ID accepted.
Passport
Driver License
Medicaid card with DOB
School ID with DOB
• We serve our patients on a first come – first served basis. We tend to get busy in the afternoon, so we recommend patients arrive as early as possible. If we are unable to serve a patient on the day of arrival, we will do our best to give them priority on the following day of service.
• We are not a COVID 19 testing site.
