I am Dr. Dakota Boisseau, a board certified family medicine physician and owner of River Direct Primary Care.

GLP-1s work. But are you losing fat, or muscle?

The number on the scale is going down. But here’s what most people aren’t asking: what exactly are you losing?

Studies show that up to 15-40% of weight lost on GLP-1 medications like semaglutide and tirzepatide can come from lean muscle mass, not just fat.

Muscle isn’t just for the gym. It drives your metabolism, protects your joints, keeps you strong as you age, and helps your body stay insulin-sensitive. Lose too much of it, and you can end up lighter on the scale but metabolically worse off and at higher risk of regaining the weight as fat if you ever stop the medication.

The good news? This is largely preventable.

The two most evidence-backed strategies:

Brain fog during menopause is real and it’s one of the most common symptoms nobody warned you about.

Forgetfulness. Trouble concentrating. Feeling like you’re moving through mud.

More than two-thirds of women experience this during the menopause transition, and most assume the worst.

Here’s what the research actually shows: your core cognitive abilities are intact. The brain fog is real, but it is not permanent damage. It is a hormonal and lifestyle-driven symptom which means there are real things you can do about it.

Start here:
∙ Prioritize sleep above almost everything else. Hormonal sleep disruption makes cognitive symptoms significantly worse.
∙ Move your body daily. Aerobic exercise is one of the most evidence-backed tools for brain health during this transition.
∙ Eat to support your brain. Blood sugar stability, protein, and omega-3s matter more than you think right now.

A question I’ve been hearing more often lately: “Can I just get a blood test for Alzheimer’s now?”

The short answer is yes — and no.

Earlier this year, the FDA cleared the first blood test for Alzheimer’s assessment in primary care settings. It’s now available nationwide for patients 55 and older who are experiencing symptoms of cognitive decline.

For a long time, evaluating cognitive symptoms meant referrals to specialists, PET scans, or spinal taps. A simple blood draw that can be done in your doctor’s office changes the conversation.

But here’s the part that tends to get left out of the headlines: this test is designed to rule out amyloid plaques — a hallmark of Alzheimer’s disease. It is not a standalone diagnostic tool, and a positive result is not a diagnosis.

Blood tests must be interpreted in clinical context and confirmed with additional testing when treatment decisions depend on the result.

And perhaps most importantly: if your memory occasionally slips but you’re still managing your medications, finances, and daily activities independently, routine biomarker testing is not currently recommended and may cause more harm than good.

The right question isn’t “can I get this test.” It’s “should I get this test — and what would we do with the result?”
