Our rehabilitation programs are centered around the individual needs of each patient to improve health, function and quality of life. Acute Inpatient Rehabilitation Facilities differ fro

m other post-acute options most notably in the amount of therapy offered and medical oversight provided. We offer specialized therapy, rehab-trained nursing care, rehabilitation physician involvement and disease state management tailored to our patient population. Sam is truly solution-focused, always looking for ways to overcome challenges and find the best outcomes for our patients, families, and team members. Her dedication extends beyond her daily responsibilities, as she continues to support our Stroke Support Group even after transitioning to a new team.

Sam is frequently recognized in patient surveys and reviews for her creativity, compassion, and willingness to help others. Patients and families consistently appreciate the positive impact she makes during their rehabilitation journey.

In addition, Sam continues to maintain and grow the Leisure Activity Project she originally created as a student here, demonstrating her ongoing commitment to enhancing the patient experience.

Our June Stroke Support Group was filled with celebration, education, and inspiration!

Today, we recognized one of our stroke survivors as they celebrated their one-year stroke-free anniversary—a remarkable milestone and testament to their hard work, perseverance, and recovery journey.

The group also learned about neuro outpatient services available in our community and discussed Vivistim, an innovative technology designed to help stroke survivors improve upper extremity function and continue progressing in their recovery.

đź’ś Recognizing Stroke Awareness Month đź’ś

We are proud to share Mohinder Kaur’s inspiring stroke recovery journey.

When Mohinder was admitted to Community Rehab Hospital following a stroke that affected the left side of her body, she required the assistance of two caregivers for all self-care and mobility tasks. Her left arm had no functional movement, and everyday activities presented significant challenges.

Through determination, hard work, and dedication to her therapy program, Mohinder made tremendous progress. By the time of discharge, she was able to walk 50 feet with a walker and only minimal assistance. She regained the ability to use her left leg for mobility and showed emerging movement in her left arm, allowing her to better use her hand while walking with her walker. She also progressed to needing only minimal assistance with self-care activities.

Mohinder’s success is a powerful reminder that stroke recovery is possible. Her journey demonstrates that with perseverance, support, and a commitment to rehabilitation, individuals can overcome challenges regardless of language, cultural, or cognitive barriers.

We are honored to have been part of her recovery and wish her continued success as she enjoys being back home with her family.

In recognition of Stroke Awareness Month, we are proud to share Donna’s inspiring recovery journey.

When Donna was admitted to Community Rehab Hospital following a stroke, she experienced significant weakness on her left side along with severe left-sided neglect. Upon admission, she required the maximum assistance of two people for bed mobility, transfers, and showering, and she was unable to walk.

In addition to the challenges from her stroke, Donna was dealing with other aches and pains that sometimes made participation in therapy difficult. Despite these obstacles, her determination never wavered.

Through the persistence of her therapy team and Donna’s strong desire to return home walking, she made remarkable progress. By discharge, she had advanced to requiring only supervision for transfers and was able to walk household distances using a walker. She also became increasingly independent with her daily activities.

Donna worked diligently with speech therapy to improve her cognition, memory, and problem-solving skills following her stroke. Her hard work and dedication led to a successful discharge home with her family.

We celebrate Donna’s perseverance and are honored to have been part of her recovery journey. Her story is a powerful reminder of what can be achieved through determination, support, and rehabilitation.
