Lip enhancement should be tailored to the patient’s facial features, anatomy, and desired level of fullness.

This 22-year-old patient wanted fuller lips while preserving a natural appearance. Lip augmentation was performed using one syringe of Juvéderm Ultra. The result adds volume and definition while maintaining the patient’s natural beauty. The amount used in this case reflects her individual treatment plan and should not be viewed as a recommendation for every patient.

How long do plastic surgery results actually last? The answer depends on the procedure and on the changes your body goes through afterward.

Rhinoplasty results are generally permanent because the underlying bone and cartilage are reshaped. Breast surgery can provide long-lasting results, but pregnancy, breastfeeding, weight changes, and aging may affect the breasts over time. A facelift can last 10 years or more, although it does not stop the natural aging process. Tummy tuck results are designed to be long-lasting, but major weight fluctuations or pregnancy can change the outcome.

Sometimes, the best surgical decision is to say no.

I would not perform a facelift on a patient who is actively smoking, as smoking and ni****ne can impair blood flow and wound healing, making a procedure like a facelift significantly riskier. I would also decline a breast augmentation request for implants that are disproportionately large for the patient’s anatomy. Implant selection should account for the chest, skin, and breast tissue, as well as the desired appearance.

An honest consultation includes a discussion of risks, limitations, and alternatives. My responsibility is to recommend a plan that I believe is safe and appropriate for the individual patient, even when that means postponing surgery or not proceeding.

Revision rhinoplasty can bring up very different questions from a first rhinoplasty.

Before discussing another operation, I want to understand your previous procedure, what concerns you now, and what you hope to achieve. I assess the nasal structure, scar tissue, cartilage support, and breathing, then explain which changes may be possible and where the limitations may be. Revision nose surgery requires a plan built around the anatomy that is present today. For some patients, the priority is contour. For others, it is restoring support, improving symmetry, or addressing function.

Rhinoplasty should refine the nose in a way that works with the patient’s own features.

This 29-year-old patient underwent rhinoplasty with septal cartilage grafting to address a broad, bulbous nasal tip, wide nasal bones, an uneven bridge, and a small dorsal hump. During surgery, the dorsal hump was reduced, the nasal bones were repositioned to create a narrower framework, and the nasal tip was carefully sculpted for improved definition. The result shown here is a smaller, smoother, more balanced nasal appearance that remains natural to her face.

Why are some plastic surgery results immediately obvious while others are difficult to detect?

Often, the difference comes down to planning, restraint, and respect for the patient’s anatomy. My approach is conservative: the goal is not to create the same nose, face, or body for every patient, but to make carefully measured changes that improve balance while preserving individual features.

This is especially important in rhinoplasty. A well-planned result should look refined and proportionate without making the patient look like a different person.

The best result is one that feels natural to your face and still looks like you.

Three questions I always ask before rhinoplasty, and they matter just as much as the technique itself.

1. What specifically bothers you about your nose? I don’t lead with my own opinion. I want to understand exactly what you see and what you want to change.
2. How well do you breathe through your nose? Function matters just as much as form, and any airway concerns need to be factored into the plan.
3. What are your expectations for the result? This is where we talk honestly about what is realistically achievable for your anatomy, so the outcome aligns with what you actually envisioned.

These conversations are where the real planning begins, long before we ever step into the operating room.

Abdominal wall reconstruction is planned around the structural problem beneath the skin.

This patient underwent repair of a ventral hernia. The before-and-after photographs show the abdomen before surgery and after reconstruction of the abdominal wall. The surgical goal was to repair the hernia and restore support to the affected area, with the visible improvement representing one part of the overall result.

Rhinoplasty requires careful attention to both aesthetics and structural support.

This 28-year-old patient underwent rhinoplasty with septal cartilage grafts to improve a prominent dorsal hump, hanging columella, and limited tip definition. During surgery, the nasal bridge was reduced, the hanging columella was corrected, and the nasal tip was carefully sculpted. Septal cartilage grafts were used to reinforce the middle vault, nasal tip, and nasal airways, while also creating smoother, straighter, and more symmetrical dorsal aesthetic lines. The result is a refined, naturally proportioned nose with improved definition and structural support.
