Exceed HHC, a pediatric home care agency in New Jersey currently has a position for full time, part time and per diem experienced and compassionate LPNs to join our team in Bergen, Passaic, Morris, Hudson, and Essex Counties.
The LPN will assess and follow the plan of care for patients in the home that they are assigned to. The LPN will promote continuity of safe and appropriate care through their work.
$25/hr to $30/hr
Medical and dental insurance provided
Essential Functions:
Provides direct one on one care to medically fragile children in the comforts of their homes. This can include such skills such as Peg tube management, foley catheter care, tracheostomy's care, supplemental oxygen therapy, wound care, and medication administration. In some instances ventilators.
Promotes personal safety and a safe environment for clients and coworkers.
Performs job duties in accordance with agency policies, procedures, and professional and community standards.
Performs other job duties as assigned.
Key responsibilities:
- Provide one-to-one skilled care in the home / school setting
- Lifting / repositioning at times
- Completing daily charting
- Follow plan of care
- Following all clinical and office policies
Job Types: Full-time, Part-time, Per-Diem
Job Requirements:
- Graduate of an accredited school of licensed professional nursing.
- Active New Jersey LPN license
- Minimum of two (2) years of pediatric nursing experience preferred.
- Prior Home care experience preferred.
- Effective written and oral communications skills and good interpersonal skills.
- Organizational and time management skills.
- Committed and reliable in regards to assigned shifts
