Our Community Health Workers are on the front lines, building trusted relationships and bridging the gap between our communities and RWJBarnabas Health. They help patients and their families navigate programs and services, follow their care plans and access the resources they need to live as healthy a life as possible.

Through early intervention, education and compassionate support, they help improve health outcomes and strengthen our communities.
