You know me at the clinic. Let me introduce you to another part of me.

Before the first patient, before the first email, I am outside in whatever I am already wearing, opening the coop with 30+ hens pressed against the door like I have been gone a year. They come running the second they see me. Then it becomes a competition over who gets my arms and who gets my lap, and somebody is always sitting on my shoe. Occasionally I am dressed for a lecture and doing it anyway, because the chickens do not wait and neither does the day.

They consider me staff, and I accept that. They do not care about my credentials nor my curriculum vitae. They care that I show up to feed them and that I love them, and that is the entire job description.

This is where the noise stops. A farm keeps a very simple kind of time. The coop opens at first light whether or not I slept. The water gets filled. The eggs get collected, still warm, still dusty, bloom intact. Nothing about it can be optimized or rushed, and that is exactly why I need it. You cannot hurry a hen, and standing in the grass with one tucked under my chin is the only place my brain goes completely quiet.

For those that do know me; heels in the hen house is not a surprise. 🥰

I have built three businesses. My nervous system kept the receipts.

For years I measured success the way most of us were taught to measure it. Revenue, growth, bookings, the next credential. I hit the numbers. I was also sleeping five hours a night, running on cortisol, and calling it discipline.

Here is what I understand now, as a clinician and as a woman who learned it the hard way. Chronic stress is not a personality trait. It is a physiologic state, and it bills you eventually. Sleep architecture, fat distribution, skin quality, hormone balance, and the rate at which your face ages are all downstream of a nervous system that is never permitted to stand down.

Ambition is not the problem. Operating without regulation is.

Build the practice. Regulate the person running it.

1. Watch my hands between passes. I clean before every single pass. You will never see me touch the cannula or the introducer with my gloves. Providers reduce the risk of infection by maintaining the strictest hygiene practices. If you see a provider touching the needle, wiping the needle on gauze, or handing the patient an iPad to sign consent while gloved, RUN!

2. Watch where the product goes. It is placed underneath the jaw, not on the side. Filling the sides will widen her. Hyaluronic acid placed underneath lengthens the profile, borrows lax tissue from the submental region, and can help reduce the appearance of that pesky bulk under the chin that develops no matter our weight.

3. Watch the pace. Notice how slow and intentional each stroke is. The video is sped up, and it is still rhythmic. That rhythm is what gives me the control to stay in the intended plane and to decrease the patient’s discomfort. Speed also hurts the patient, good treatments take time.

4. Watch what happens at the end. I assess the patient at rest and in animation after treatment. This should be done before and after, and still photographs must be taken.

These are just a few of the nuances patients can look for when choosing a provider, and I hope providers take something from it as well. Every day I am learning and growing, and yes, that means I am constantly changing and evolving my thoughts and my techniques. As one of my colleagues said the other day It’s Tara 2.0 💕

I will always honor my scientific inquiry and lead with evidence for my patients safety.

No agenda. No hustle. Nothing to optimize.

Just a small brown bunny, a pile of greens, and absolutely nowhere else to be.

We spend most of our lives convinced that perfect is something we have to build. Something earned, engineered, polished, posted. And then you stand still for ninety seconds in the grass and realize it already happened this morning, quietly, without any help from us.

Sometimes perfect is not something you create. Sometimes perfect is just sitting back and nsture happens.

Why did you choose to be a medical aesthetics provider?

What is the legacy you want to leave in this profession?

Before we embrace anything new we must ask ourselves why am I here, what do I hope to gain, how will this positively impact my life or my patients life.

I had the pleasure of leading an MD Codes program in Austin Texas last week. I opened my session with asking the attendees their why. I was thrilled to see so many pens immediately start to zip across the pages of their notebook. That immediacy showed me I was in a room full or medical providers on a mission to do better, be better and that where questioning their current practices.

This thoughtful group of providers embraced the philosophy of the MD codes and was so invested in learning.

I’m grateful to have had the opportunity to lead the program but even more grateful to have had the opportunity to share the room with such a lovely and inspiring group of professionals.
