💼 What You’ll Do
As a key member of our rehabilitation team, you’ll:
✔️ Assess client needs and build personalized support plans
✔️ Provide hands‑on guidance in homes, communities, and work sites
✔️ Collaborate with a multidisciplinary team
✔️ Lead meaningful skill‑building activities
✔️ Track progress and adjust support as needed
✔️ Educate and empower clients & families
✔️ Advocate for client needs in the community
✔️ Provide occasional transportation (with mileage reimbursed at IRS rates)

🎓 What We’re Looking For
📘 Bachelor’s degree in human services, rehabilitation, social sciences, or related fields
—or—
📗 Associate’s degree + 1 year experience
🚗 Valid driver’s license + reliable daily vehicle access
📅 Strong organization & follow‑through
💬 Excellent communication skills
🤝 Team‑oriented and compassionate
🧠 ACRE certification (completed after hire)
