A “precise pocket” can look perfect on day one and still fail long-term if the implant has no ligament support.

Yes, you can create a pocket under direct visualization that fits “hand in glove.” The question is what happens years later when tissues stretch and gravity starts doing its job. That’s when you see implants drift laterally, especially when a patient is lying down.

This is where Motiva Preservé breast augmentation is fundamentally different. Preservé is designed to preserve Cooper’s ligaments, allowing them to surround the implant and create a stable “nest” of support that persists over time.

And the data matters. A recent series of 100 consecutive Preservé patients followed for 3 years reported zero capsular contracture, zero lower malposition, zero lateral malposition, zero hematomas requiring reoperation, and zero loss of ni**le sensation. It’s hard to beat zero.

Capsular contracture is one of the biggest concerns in breast augmentation and especially in Motiva Preservé breast augmentation discussions, so let’s be precise.

The published literature doesn’t point to “trauma” as the primary driver of capsular contracture. Surgery is, by definition, a controlled form of trauma. What the papers consistently associate with higher contracture risk is blood left around the implant from postoperative bleeding.

That’s why my focus in Preservé is meticulous planning and a bloodless, controlled pocket. When bleeding is minimized and managed properly, the risk profile improves and outcomes stay more stable long term.

Wide awake breast augmentation is possible, but it’s the Preservé approach that makes it a truly viable option for the right patient.

Because Preservé is designed to be minimally invasive and tissue-preserving, many patients can have a calm, controlled experience with local anesthesia and remain awake or lightly sedated. It’s not the majority of cases, and it’s never something I push, but for women who are anxious about being fully “put to sleep,” it can be a game-changing option.

Wide awake surgery is getting a lot of attention, but here’s the nuance.

If you’re doing a simple implant exchange with no pocket work, that can often be done awake. But once you’re talking about a true revision new pocket creation, repairs, or a lift that’s a much bigger operation, and the real question becomes the patient’s comfort and overall experience.

Also important: there’s no evidence that general anesthesia increases dementia risk.

My goal is always the same: choose the safest, most controlled plan and the one that gives the patient the best experience, not the most “viral” concept.

When it comes to breast augmentation scars, location and tension are everything.

An incision that heals in the natural breast crease tends to have very little tension, which is exactly what you want for the best cosmetic scar. If the crease drops during healing, the scar can ride higher on the breast and won’t look as refined.

One of the things I’ve appreciated with Motiva Preservé is how consistently the lower breast crease stays where it belongs, which helps keep the scar hidden and optimized from the outset.
