🏆 Mason District Hospital Receives Care Management Award

Mason District Hospital has been recognized by the Illinois Rural Community Care Organization (IRCCO) with the Best in Accountable Care Organization (ACO) Care Management Award.

This award recognizes the work our care management team does every day to provide proactive, patient-centered support and help patients manage chronic conditions, stay connected to their care, and access the services and resources they need.

Since our Chronic Care Management (CCM) program began in 2018, our team has provided personalized, one-on-one support to help patients navigate their healthcare, coordinate care between providers and specialists, and feel more confident managing their health.

“Our CCM nurses become our patients’ personal nurses, someone they can call directly when they have questions, need assistance, or simply need someone to talk to,” said Amy Ewalt, Director of Quality, Infection Prevention, Employee Health & Case Management.

We’re especially proud to recognize our clinic care coordinators, Lizzie and Aimee, who provide this important support to our patients every day.

This award is a reflection of the dedication of our entire team and the importance of working together to make sure our patients receive the right support at the right time.

Congratulations to everyone who plays a part in making our Chronic Care Management program a valuable resource for the patients we serve! 👏

“I’ve been enrolled in Mason District Hospital’s Chronic Care Management (CCM) program since 2019, and I honestly don’t know what I would do without it. It has made a big difference for me over the years.

I have a hard time putting my continuous glucose monitor sensor on myself, so being able to go to CCM for help with my sensor changes is very important to me. There have even been two or three times when my sensor came off and the care team stopped by my house on their way home. I was so pleased that they would go out of their way to help me.

The CCM team has also helped coordinate my care between my primary care provider and specialists. I really appreciate knowing that I have someone I can turn to when I need help.

I would recommend Chronic Care Management to anyone living with ongoing health conditions. Having a team that is there for you makes all the difference.”
