🏆 Mason District Hospital Receives Care Management Award
Mason District Hospital has been recognized by the Illinois Rural Community Care Organization (IRCCO) with the Best in Accountable Care Organization (ACO) Care Management Award.
This award recognizes the work our care management team does every day to provide proactive, patient-centered support and help patients manage chronic conditions, stay connected to their care, and access the services and resources they need.
Since our Chronic Care Management (CCM) program began in 2018, our team has provided personalized, one-on-one support to help patients navigate their healthcare, coordinate care between providers and specialists, and feel more confident managing their health.
“Our CCM nurses become our patients’ personal nurses, someone they can call directly when they have questions, need assistance, or simply need someone to talk to,” said Amy Ewalt, Director of Quality, Infection Prevention, Employee Health & Case Management.
We’re especially proud to recognize our clinic care coordinators, Lizzie and Aimee, who provide this important support to our patients every day.
This award is a reflection of the dedication of our entire team and the importance of working together to make sure our patients receive the right support at the right time.
