One of the most common things people notice during a massage is that their nose suddenly feels “plugged up” when they’re lying face down. Most people assume it’s pressure from the face cradle or that the massage is “moving toxins,” but the explanation is usually much more straightforward and anatomical.

Inside the nose are structures called turbinates. These are curved shelves of tissue lined with blood vessels whose job is to warm, humidify, and filter the air you breathe. The tissue inside the turbinates naturally expands and shrinks throughout the day as blood flow changes.

When you’re upright, gravity helps keep blood distributed lower in the body. But when you lie face down — especially with your head level to or below your heart — blood flow shifts more toward the head and neck. This can cause the blood vessels inside the turbinates to temporarily enlarge, narrowing the nasal airway and creating the sensation of congestion.

At the same time, mucus that normally drains toward the back of the throat while upright may shift forward because of your position. That’s why some people feel the need to blow their nose after turning over during a massage.

In most cases, this is completely normal and temporary. The massage itself is not creating mucus or “releasing toxins.” What people are experiencing is usually positional nasal congestion — a normal response of the nasal tissues and circulation to body position and gravity.
