Recently, members of the Oakland Police Department met with staff from Horizon’s Diversion Triage Center to learn more about how the program supports individuals experiencing substance use, mental health, and other complex challenges.

What makes the Diversion Triage Center different is the continued support provided after initial contact. Our peer specialists work alongside clients through paperwork, coaching, resource navigation, and follow-through, helping reduce barriers and strengthen connections to care.
