Missing early Lyme Disease isn't just a delayed diagnosis.

These are the 4 rules I follow when a patient's test comes back negative but the clinical picture doesn't match.

Because in my experience, a negative test is not the end of the conversation.

It's the beginning of a deeper one.

Most clinicians treat Lyme Disease like a checklist.

Test. Diagnose. Treat.

But that's not how complex disease works — and after years of treating the sickest Lyme patients, that linear thinking fails them every single time.

So I stopped following the checklist.

Instead I observe. I interpret. I adjust. I reassess. Every move I make is based on the full clinical picture not just one test result.

That's the framework I use when testing doesn't give a clear answer. It's adaptive, strategic, and it gets results.

If you can't show the data, you don't have a protocol. You have a guess.

There's a version of clinical courage that I deeply respect — the willingness to try something outside the box when a patient is out of options, to document everything, monitor closely, and share your findings so others can learn.

And then there's the other version. The one where a high-dose protocol gets passed around with no safety data, no monitoring, and no accountability — just confidence and anecdote.

One of those moves medicine forward. The other quietly harms patients while the doctor tells everyone how well it worked.
