The Holyoke VNA started with one nurse making thousands of visits in hundreds of patients homes. Hospice Life Care provides compassionate care to terminally ill patients throughout the Pioneer Valley. Hospice's holistic approach to care seeks to make patients as comfortable as possible, and to manage symptoms so that their last days may be spent with dignity and quality. Hospice services are provided at home, assisted living, or in long term care facilities. Hospice addresses not only the physical dimensions of terminal illness, but also the practical, emotional and spiritual. As part of its guiding philosophy, hospice pays special attention to the needs of family members as they make this difficult journey. Hospice Life Care Provides:
•A team of professionals:
- Medical Director
- Registered Nurses
- Social Workers
- Dietician
- Chaplain
- Volunteer Coordinator
•Increased emotional and practical support for the patient and family
•24-hour/seven day availability for problems and emergencies
•Additional help with errands, as well as companionship by trained, compassionate volunteers
•Home Health Aides
•Payment for medication and equipment related to the terminal illness
•Specialized support such as physical, occupational, and speech therapy
•Thirteen (13) months bereavement support
How Hospice Works:
Hospice life Care serves patients and families throughout the Pioneer Valley. The typical patient:
•Has an estimated life expectancy of six months or less; if the disease runs its normal progression
•Is aware of the prognosis and consents to the goals of hospice care, and;
•Has a primary care physician who works with Hospice or delegates care to the Hospice Medical Director
