From coordinating Swing Bed admissions and discharge planning to connecting patients with resources and supporting their recovery journey, our Case Management team works tirelessly behind the scenes to help patients receive the care they need closer to home.

💙 Celebrating National Case Management Week! 💙

Today, we are proud to recognize Quanna Tolbert for the compassion, creativity, and dedication she brings to our Swing Bed Program every day.

As Activities Director, Quanna helps make recovery feel more like home. From planning engaging activities and spending time reading with patients to arranging haircuts and encouraging visits to our Healing Garden, she goes above and beyond to support the physical, emotional, and social well-being of those in our care.

Her thoughtful approach helps patients stay connected, engaged, and encouraged throughout their recovery journey. The small things she does each day often make the biggest difference.

Thank you, Quana, for the kindness, joy, and personal touch you bring to our patients and their families. ❤️🏥
