As a physician, here's the honest conversation I have with every GLP-1 patient who stops losing weight.
The scale stopped moving. You're still taking the medication. And you're wondering if something is wrong with you.
It's not you. Here's what's actually happening.
Research published in Obesity (2024) confirms that GLP-1 weight loss plateaus are a normal physiological response — not a sign of failure. Your basal metabolic rate slows as you lose weight, and your body begins to compensate. The medication is still working. Your biology has just adapted.
But here's where most providers stop — and where I go deeper:
→ Diet quality still drives outcomes. GLP-1 suppresses appetite, not consequences. Fast food and vacation eating can stall progress even at a full therapeutic dose.
→ Sleep is metabolic medicine. Poor sleep raises ghrelin (hunger hormone) and cortisol (fat-storage hormone). If you're working nights or sleeping poorly, I want to know why — and hormones are often the answer.
→ Hormone optimization unlocks the ceiling. For patients who've maxed out on GLP-1 dosing, getting testosterone, thyroid, and cortisol in range has changed everything.
→ And sometimes, a structured medication break is the strategy. After 2+ years, some patients develop receptor desensitization. A supervised pause can restore sensitivity. It's not quitting. It's medicine.
You deserve more than "increase the dose." You deserve a plan.
