At Fountain Springs at Cape May, our rehabilitation program is built on strength, compassion and results. We also value the strong relationships we have built with our local hospital partners and case managers at Cooper University Hospital Cape Regional. By working closely together, we help ensure smooth transitions of care, clear communication, and the best possible outcomes for patients and families. Our collaborative approach allows us to support the community with trusted rehab rehabilitation service services when they are needed most.
In addition, we have a monthly lunch scheduled with our clinical liaison, Pamela Ireland, which helps us maintain open communication, strengthen partnerships, and continue, providing the highest level of coordinated care.
