She had felt this way since she was a teenager. She had the surgery at 39.

But not because she went back and forth on it. Because she wanted to be finished having children first. Pregnancy and breastfeeding can change breast size and shape after a reduction, sometimes enough to undo the result. Plenty of patients decide that trade is worth it and move forward earlier. This one chose to wait.

By the time she came in, she was a mother of two at 5'4" and 130 pounds, and she wanted breasts that fit her frame and sat higher. Dr. Cash removed 370 grams from the right side and 295 from the left. That gap between the two numbers is deliberate. She came in asymmetric and wanted to leave even.

How much does "too heavy" actually weigh?

For this 47-year-old from Humble, about five and a half pounds! That's what came off: 1,322 grams from the right breast, 1,163 from the left. Roughly a newborn, carried on her chest through every workout and every night's sleep. Wow!

She told us exercise had stopped being worth it and that getting dressed had become a negotiation. Neither of those shows up on a chart, but both are why patients come in.

Dr. Cash used an inferior pedicle reduction, where the blood supply and nerve supply to the ni**le come from below. It's a workhorse for larger reductions and it holds its shape.

Three kids, a 5'6" frame, 124 pounds, and an abdominal wall that needed structural repair.

Diastasis this wide doesn't close with core work. This 39-year-old came in with a significant separation, dense stretch marks through the center, and thin skin over the top. Dr. Cash told her at consultation that mesh would likely be part of the plan, and it was. The repair used mesh at three sites, including an old tubal ligation defect below the navel, along with repair of the hernia contents. 262 grams of skin and tissue came off.

These photos are at one year, and she is within a pound of her starting weight. That's the part people miss. A tummy tuck repairs the abdominal wall and removes loose skin, it's not simply a weight loss procedure.

This 41-year-old came in at 5'3" and 169 pounds. Dr. Cash repaired the hernia, plicated the muscle, and removed 1,312 grams of skin and tissue, then took 1,450 mL from each flank with liposuction to sharpen the waistline. She is 160 pounds in the after photo, taken six months out.

Hernia repair and a tummy tuck pair well because we're is already working in that layer of the abdominal wall. Doing both at once means one operation and one recovery instead of two.

Her hysterectomy and her tummy tuck happened in the same operation.

This 41-year-old had a long history of anemia, uterine fibroids, heavy bleeding, and endometriosis, and she was already scheduled for a full hysterectomy with her OB/GYN. Dr. Cash operated alongside that surgeon and performed the tummy tuck during the same case. One round of anesthesia, one recovery, instead of two of each and separate recovery times.

Combined cases take real coordination between both teams and they're not the right call for every patient. When the timing lines up, it can save someone months. This after photo is at her nine month follow-up.

This patient wanted better balance between her breasts, plus removal of ectopic breast tissue and loose skin under her arm that CoolSculpting hadn't been able to address.
