✨ Not every blepharoplasty is simply about removing skin.

In this case, my patient had prolapse of her lacrimal glands causing the lateral bulge. In order to achieve a smooth and natural result, her lacrimal gland had to be repositioned inside the orbital rim, in addition to the upper lid skin removal .

Beautiful eyelid surgery is about being familiar with the delicate eyelid anatomy, knowing what to remove, what to preserve, and what to reposition. The goal is never simply “less”—it’s a natural, refreshed, and rejuvenated result. 👁️✨

Does weight loss affect blepharoplasty planning?

For upper blepharoplasty, generally no—weight loss typically doesn’t significantly affect the eyelid skin or orbital fat.

For lower blepharoplasty, yes. Rapid weight loss won’t affect the amount of lower lid fat prolapse there is, but it can cause volume loss in the cheeks and midface, which can change our surgical approach—how much fat we remove versus preserve and reposition into the cheek to restore lost volume.

So if you’re considering a lower blepharoplasty, ideally, you should be at or near your target weight before your surgical consultation. 💫

This patient presented to me after an outside lower blepharoplasty that resulted in lower lid retraction. Her lower lids were pulled downward, causing scleral show, redness, irritation, tearing, and an unnatural contour.

We initially treated this with a series of anti-scar injections combined with upward massage, followed by revisional surgeries to restore proper lower lid position and support. Her result is an improved lid position, enhanced comfort, and a more natural appearance. 💛

Blepharoplasty requires precise technique and deep respect for the delicate anatomy of the lower eyelid. Complications are always possible with any surgery, but but always do your research when choosing your eyelid surgeon. Oculoplastic surgeons are specialized ophthalmologists who undergo two years of fellowship focused specifically on plastic surgery around the eyes. 💫

Healing Phases of Upper Eyelid Blepharoplasty 💛

Pictured is my beautiful patient before surgery, 1 week after, and 2 months following upper lid blepharoplasty. The transformation continues to evolve as healing progresses!

Bruising and swelling typically peak within the first 48 hours after surgery before gradually improving. While healing timelines vary based on individual factors, you can help optimize recovery by:
• Avoiding blood thinners (as medically appropriate)
• Not smoking
• Applying cold compresses during the first few days
• Minimizing strenuous physical activity during the first postoperative week

Most swelling resolves by about 3 months, although complete healing can take longer in some patients. The incision is carefully placed within the natural upper lid crease, where it’s hidden with the eyes open. It’s common for the incision to appear pink and raised in the first few months, and this gradually resolves over 3–6 months.
