We've got occupational therapy at home!

If you or someone you know is having trouble navigating daily routines, Modern Homecare brings the clinic straight to your home.

Occupational therapy (OT) is a great way to get back to the actual activities of daily living (ADLs) that keep you functional. This could look like taking a shower again on your own, getting dressed, or cooking a meal without exhausting yourself. Our therapists help you adapt your routine and figure out the right home safety modifications.

Like most other home health services, you need a doctor's order because home healthcare always starts with the approval of your primary care physician or discharging doctor writing a referral and certifying your plan of care.

You must also be "homebound". This doesn’t mean you can never leave the house! It simply means leaving your home requires a considerable, taxing effort or is medically unsafe.

It pairs with other services! Under Medicare rules, OT cannot be the only reason you initially start home healthcare. It must be paired with skilled nursing, physical therapy, or speech therapy at first. However, OT can keep going even after those other services wrap up.

It’s customized to your actual home. Instead of practicing in a facility, our therapists work with your actual environment to figure out exactly what adaptive tools or techniques you need to navigate your space safely. We do offer free assessments and consulting before your care begins.

If you're wondering if you qualify for our occupational therapy services in the Greater Houston and surrounding area, please reach out to us directly! Medical professionals are definitely encouraged to reach out to us for thorough information on our services!

We’d love to answer your questions and help you understand care of home health.

When exploring Personal Assistance Services (PAS), the initial focus is usually on practical needs like cooking, cleaning, or mobility support. However, the true benefit of non-medical in-home care extends well beyond checking off a daily to-do list. If you are looking to restore balance to family relationships or you need a reliable safety net to live and do not have local support, having a professional caregiver can fundamentally change your daily life.

From a health and safety standpoint, caregivers actively reduce many risks by keeping the home organized. They prepare well-balanced meals that help manage health conditions, assist with hygiene to prevent infections, and monitor subtle changes in mobility or appetite before they become emergencies. Emotionally, regular companionship combats senior isolation, keeping individuals engaged and connected to their existing communities.

Most importantly, home care allows individuals to preserve their autonomy without the potential decision of moving into a facility. Caregivers are trained to provide hands-on support without taking over completely (unless needed). This gives family members immediate relief from full-time caregiving and provides those handling care on their own the exact support system they need to safely maintain their household.

PAS is also a practical financial option, often proving more cost-effective than residential facilities. We offer structured weekly care packages ranging from 20 to 60 hours, which allows for predictable budgeting and dedicated scheduling. Every care plan begins with a complimentary in-home assessment to evaluate specific needs and ensure we build a plan that works best for you!

Please reach out to us directly to discuss these benefits in detail! Our goal is to make sure every family and individual understands their care and our services.

Houston! If daily routines are getting difficult to manage alone, Modern Homecare can help!

Our Personal Assistance Services (PAS) provide hands-on, non-medical support for people who want to stay safe and independent at home but don't need skilled nursing.

Our caregivers assist with things like light housekeeping, meal preparation, medication reminders, personal hygiene, and transportation to doctor's appointments or the grocery store (includes other errands as well!).

We offer structured weekly schedules starting at 20 hours a week up to 60 hours a week, as well as overnight care. Everything starts with a free in-home assessment so we can figure out exactly what kind of help makes sense for your situation.

We will be sharing more details about these services as the week goes on. In the meantime, we are available by phone or email to answer any questions you may have.

Give us a call at (281) 501-0350 to schedule a complimentary in-home assessment, or send referrals directly to [email protected].

Taking five or more prescription medications a day is known as polypharmacy. While each drug treats a specific issue, taking them all together increases the chance of harmful interactions and side effects. This can sometimes lead to a "prescribing cascade," where new medications are prescribed just to treat the side effects of the original ones.

Taking multiple medications at once can overwork the body's natural filtering systems. As we age, the liver’s size and blood flow decreases, reducing the body’s ability to clear out medications. At the same time, the kidneys lose a portion of their capacity to filter waste because of these physical changes, processing several drugs puts continuous stress on the organs, making careful dosage adjustments essential to prevent toxicity.

Managing this daily medication burden affects both physical balance and mental clarity. Studies show that taking five or more medications is strongly linked to memory issues, as well as a higher risk of falls and fractures that limit mobility.
Understanding how these medications interact is key to maintaining independence at home. The combined effect of multiple drugs can cause extreme fatigue, loss of appetite, and sudden mood shifts. It is also tied to emotional impacts like social isolation.

Ultimately, heavy medication routines can make it difficult for individuals to safely perform basic daily activities on their own.

Keeping track of multiple prescriptions is a lot to manage, especially when side effects start blurring into everyday life. If you have questions about keeping up with medication management like side effects or prescription adjustments, please, always consult your doctor directly for medical advice and ongoing medication management!

As we get older, and watch our own parents age, we start noticing when the basic daily routines become a struggle. They might not be sick or need a registered nurse, but it becomes obvious they shouldn't be at home alone anymore. It’s a tough transition for any family. You want to respect their independence, but you also need to know they are safe when you can't be there.

Allowing a professional that you trust step in for those daily tasks doesn't take away the sense of self reliance, it just helps them keep it with a little assistance. It allows them to safely stay in the home they love without the constant outside worry.

There is no shame in not being able to manage their care on your own. Arranging for some daily support just means you're making sure they get the consistent professional care, while giving yourself a much needed chance to breathe.

Advocating for yourself or a loved one starts with asking the right questions!

A good provider will welcome these conversations and take the time to make sure you feel completely confident in your choices.

Remember, you are the most important part of the decision-making process, never hesitate to ask the extra questions!

if you have an aging parent or relative, 🔁 and 👍 this post. You will want to reference this later!

We see it all the time, patients are surprised by how much support is actually available to them beyond skilled care!

Because there is so much confusion, we wanted to lay out what one of our most important (and least understood) team members does:

1. Assessment and Planning

- Evaluating a patient's living situation, family support system, emotional state, and financial health to identify any barriers to their recovery.

- Working directly with doctors and nurses to build a realistic treatment plan that actually fits the patient's home life and capabilities.

2. Mental Health and Counseling

- Stepping in during emergencies, such as sudden health declines, suspected elder or child abuse, or severe family conflicts regarding care.

- Providing individual or family counseling to help patients process a difficult diagnosis, trauma, or the loss of physical independence.

- Identifying when family members are experiencing severe burnout and securing respite care or support groups for them.

3. Financial and Resource Navigation

- Helping families untangle Medicare, Medicaid, and private insurance paperwork to figure out what is covered.

- Tracking down local grants, food delivery services (like Meals on Wheels), and utility assistance programs.

- Coordinating the delivery and setup of necessary durable medical equipment, like hospital beds, oxygen tanks, or wheelchairs.

4. Discharge and Transition Logistics

- Organizing the safe transition of a patient from a hospital to their house, an inpatient rehab center, or a skilled nursing facility.

- Acting as the central point of contact to ensure the primary care doctor, specialists, home health nurses, and family members are all sharing the same information.

5. Legal and Advance Directives

- Guiding families through the completion of advance directives, including Living Wills, Medical Power of Attorney, and Do Not Resuscitate (DNR) orders.

- Explaining hospice and palliative care options so families can make informed decisions about comfort care.

There is a lot more to home healthcare than just the medical treatments. Having a professional step in to handle the logistics changes everything!
