What molecular story is the CNS telling?

This clinical data highlight is from a prospective UCSF study of patients with metastatic breast cancer and leptomeningeal metastases (LM) and shows why looking directly at the CSF matters.

- 50% of patients with confirmed LM had molecular alterations detected by CSF NGS
- 71.4% of tested patients had CSF tumor cells detected by CNSide®

CSF and extracranial NGS showed variable concordance and discordance, highlighting the potential for CNS-specific molecular insights that may not be captured outside the CNS. CNSide identified tumor cells and pathogenic/actionable mutations in patients with negative CSF cytology.

The latest NCCN Clinical Practice Guidelines in Oncology for Central Nervous System Cancers continue to recognize the importance of cerebrospinal fluid (CSF) analysis in the diagnosis and management of leptomeningeal metastases (LM).

For patients receiving intrathecal therapy, the updated guidelines recommend serial CSF cytology every 4-8 weeks, alongside MRI and clinical evaluation, reinforcing the value of longitudinal disease monitoring.

As CSF monitoring becomes an established part of LM management, CNSide® Tumor Cell Enumeration (TCE) is designed to complement conventional cytology by providing quantitative tumor cell assessment to support diagnosis and treatment monitoring over time.

Clinical data from the FORESEE study demonstrated that CNSide more than doubled the sensitivity of tumor detection compared with conventional cytology in matched CSF samples while providing quantitative tumor cell data to support longitudinal assessment.
