Fibromyalgia (FM) is complex, but non-restorative sleep is a consistent feature and driver of pain for FM patients. 
 
The thought behind this medication is that if we restore normal sleep architecture, particularly the deeper, restorative stages of sleep, then we can help improve FM pain. 
 
The sublingual formulation of cyclobenzaprine (Tonmya, FDA approved 8/2025) was designed to bypass first pass liver metabolism to work faster at a lower dose while limiting metabolites that may lead to next-day grogginess. 
 
But the price can be steep for Tonmya compared to generic oral cyclobenzaprine, so is the potential pain reduction worth the money?

Most of us get enough B6 in our diet, but some people may need additional supplementation, especially if they are at increased risk of deficiency.

Each medication has different benefits, risks, and fracture protection. Some primarily prevent bone loss, while others actually build new bone, and not every medication reduces hip fractures.

πŸ‘ Benefits: improves insulin sensitivity, neuropathy symptoms 
 
πŸ‘Ž Risks: GI upset, low blood sugar, rare risk of insulin autoimmune syndrome (IAS) 
 
πŸ’Š Dosing: generally 300-600 mg/day 
 
🧐 My take: worth a try for people with a higher risk of insulin resistance or those with diabetic neuropathy 
 
❗️However, remember that supplements are generally not as well-studied or regulated for safety and efficacy compared to prescription medications. They can be helpful adjuncts but not replacements for your prescription medications.
