Some families are offered avoidance. Some have access to OIT. Some are now offered Xolair.

Very few are given a serious conversation about sublingual immunotherapy (SLIT).

And even then, SLIT is increasingly being reduced to another single protocol: small amounts of allergen under the tongue, gradually increased over time.

That is one way to use SLIT. It is not the definition of SLIT.

This is where Nikhila Schroeder, MD, MEng experience is different. SLIT has been the focus of her treatment work for more than a decade, and over those years she has learned to treat it less like a recipe and more like a therapeutic platform.

- Pharmaceutical extracts or real foods. Sometimes both.
- Very low doses or higher ones.
- Slow progression or faster progression.
- Protection as the goal or meaningful ingestion.

A plan designed around a highly sensitive child may look completely different from one designed around a teenager who wants more dietary freedom.

𝐓𝐡𝐞 𝐩𝐚𝐭𝐢𝐞𝐧𝐭 𝐜𝐡𝐚𝐧𝐠𝐞𝐬 𝐭𝐡𝐞 𝐭𝐫𝐞𝐚𝐭𝐦𝐞𝐧𝐭. 𝐓𝐡𝐞 𝐭𝐫𝐞𝐚𝐭𝐦𝐞𝐧𝐭 𝐬𝐡𝐨𝐮𝐥𝐝 𝐧𝐨𝐭 𝐡𝐚𝐯𝐞 𝐭𝐨 𝐟𝐨𝐫𝐜𝐞 𝐭𝐡𝐞 𝐩𝐚𝐭𝐢𝐞𝐧𝐭 𝐢𝐧𝐭𝐨 𝐭𝐡𝐞 𝐩𝐫𝐨𝐭𝐨𝐜𝐨𝐥.

That is the part of SLIT I wish more people understood.

It is not “low-dose OIT.”

It is a distinct form of immunotherapy whose flexibility may be one of its greatest advantages.

I had the pleasure of speaking with Dr Ruchi Gupta, MD, MPH of Center for Food Allergy and Asthma Research, while at a previous allergy conference. She asked me to speak about a treatment for food and environmental allergies known as sublingual immunotherapy (SLIT or allergy drops).

Additional details we didn’t have time to explore is how flexible SLIT really is when used thoughtfully — including real-food formulations which are more cost-effective, different dose strategies (low vs higher dose pathways), and its uniquely strong safety profile compared with other immunotherapies. That safety opens doors not just for food and environmental allergy treatment, but for broader applications we’re actively studying and using clinically — asthma, eczema, select non-IgE conditions like FPIES, and even early food introduction. SLIT isn’t a single protocol; it shouldn't be a single protocol. It’s a platform, a foundation to build on top of for long-term benefits, not to just reach a subjective endpoint.

Understanding food (and environmental) allergies can be difficult for families. Some things that can help you on the right path...

1. Find a knowledgeable allergist who spends the time to listen to you and understand your concerns.
2. Get diagnosed properly (most likely by a board-certified allergist). The testing is just one piece of a larger puzzle, not the whole picture.
3. Know that allergies are a chronic condition that can wax and wane for your whole life, and use the tools available to improve your quality of life (QOL).
4. Change the tool when and if needed. There are many tools.
5. You don't absolutely need to eat the food you are or were allergic to in order to improve your QOL if you don't want to.

A standardized SLIT tablet for dust mite allergies that was previously available for adults is now approved for children 5-11yo. Read the article and Dr. Schroeder's thoughts at the end of the story.

"As a standardized SLIT tablet, Odactra’s FDA approval for children aged 5 to 11 years expands access to a noninvasive, disease-modifying treatment that may be covered by insurance. While Odactra lacks the dosing flexibility and multi-allergen customization of SLIT drops (and allergy shots), it provides a standardized, convenient option for children with house dust mite as a primary trigger. For families needing insurance-supported care, or for clinics without expertise in or access to customized SLIT, this offers a meaningful step forward to optimizing dust mite treatment options for children."

In February, the FDA approved the Odactra tablet, a sublingual immunotherapy, to treat house dust mite-induced allergic rhinitis in patients aged 5 to 11 years, according to a manufacturer-issued press release.As Healio previously reported, Odactra (ALK) was approved for patients aged 12 to 17 years...
