Extension drills failing? You’re skipping this step.

Missing knee extension post ACLR? It isn’t always a strength problem, sometimes it’s a tissue problem.

Before you load your patient into terminal knee extensions and quad sets, ask yourself: have you addressed what’s restricting the motion in the first place?

In this case, hamstring restriction was the limiting factor. One session of targeted manual therapy soft tissue work and dry needling!

She was cleared to return to basketball.

She wasn’t confident… so she drove 2 hours to get a second opinion.

1 year post-op ACL. Passed her hop tests. Discharged. On paper, a success story.

Here’s what we actually found:
→ 22% quad strength deficit on the surgical leg
→ Failed every eccentric task — lateral step down, forward step down, single-leg squat to box
→ ACL-RSI at 57% — her lack of confidence wasn’t in her head, it was in the data

Based on what she passed and failed, she didn’t belong in return-to-sport. She belonged back in Phase 4 — Progressive Overload, the phase most athletes clear 3–5 months post-op. She was 12 months out.
