When a loved one with dementia becomes aggressive, it can feel frightening, heartbreaking, and confusing.

But aggression in dementia is often not intentional.

Many people living with dementia lose the ability to clearly explain what they need. Instead, they may communicate through behavior.

Aggression may mean:

• Pain or discomfort
• Hunger, thirst, or fatigue
• Fear or confusion
• Too much noise or stimulation
• Feeling rushed during bathing, dressing, or care
• Infection or another medical change
• Difficulty understanding what is happening

Before asking, “Why are they acting this way?” try asking:

“What might they be trying to tell us?”

A calm voice, slower approach, simple words, reassurance, and checking for unmet needs can make a meaningful difference.

At Health Alliance SoCal, we believe dementia behaviors deserve clinical understanding, not judgment.

Families are not alone. With the right support, behavioral symptoms can often be approached with more compassion, safety, and dignity.
