After my last coronectomy post, some people concluded I coronect every wisdom tooth. 😅

Plot twist… I don’t.

This healthy 18-year-old also had a lower third molar overlapping the nerve canal on the panoramic radiograph. No CBCT was obtained. The panoramic radiograph showed well-defined canal cortication and favorable radiographic features, so a coronectomy wasn’t indicated.

✔️ Complete extraction.
✔️ No postoperative paresthesia.

The lesson? We don’t treat X-rays—we treat patients. Sometimes the safest operation is a coronectomy. Sometimes it’s a full extraction. Not every overlapping nerve case needs a CBCT, and not every overlapping nerve case needs a coronectomy.

Same surgeon. Different patient. Different decision.

Coronectomy isn’t about leaving something behind—it’s about knowing when not to remove everything.

In select wisdom teeth that sit intimately against the inferior alveolar nerve, removing the entire tooth may unnecessarily increase the risk of permanent numbness. A coronectomy removes only the crown while intentionally leaving the roots undisturbed.

➡️ Pre-op: Impacted wisdom tooth with roots closely associated with the nerve.
➡️ Post-op: Crown removed, roots intentionally retained.
➡️ 1-year follow-up: Expected root migration away from the nerve and excellent bone regeneration. The site has healed beautifully, and the retained roots remain asymptomatic.

Root migration after a coronectomy isn’t a complication—it’s often an expected part of the healing process. As the bone remodels, the retained roots may slowly move away from the nerve, making them even less likely to cause future problems.

The goal isn’t to remove the most tooth.
The goal is to provide the safest surgery with the best long-term outcome.

Sometimes, the best surgery is knowing exactly what not to remove.

🦷 What looks like “extra teeth” is actually a compound odontoma.
This patient presented with a compound odontoma in the anterior mandible. These benign developmental lesions are composed of multiple tooth-like structures and can prevent normal tooth eruption, displace adjacent teeth, or become intimately associated with the roots of neighboring teeth.
In this case, careful surgical technique was essential to:
✔️ Preserve the adjacent tooth roots
✔️ Completely remove the odontoma
✔️ Reconstruct the defect with bone graft and a resorbable membrane to support bone regeneration
Every millimeter matters. The goal isn’t just removing the lesion—it’s protecting the surrounding anatomy while creating the best environment for healing.
Many odontomas are discovered on routine X-rays before they ever cause symptoms, making regular dental exams an important part of early diagnosis.

💉 “That was it?”
One of the most common concerns we hear before oral surgery is:
“Will the IV hurt?”
The reality is that IV placement is usually quick, smooth, and over before patients know it. As oral and maxillofacial surgeons, placing IVs and administering sedation is a routine part of our training and daily practice.
At 425 Oral Surgery, we take a few moments to get you comfortable, review expectations, and answer any questions before placing the IV.
Once the IV is in place, sedation medications can be administered to help you relax throughout your procedure.
Many patients tell us afterward that the IV was much easier than they expected. 😉

🦷 Not every “extra tooth” is just an extra tooth.
An 8-year-old patient was referred to our office for delayed eruption of a permanent tooth and “odontoma”. Imaging revealed something more than a compound odontoma. We suspected a rare odontogenic tumor known as an ameloblastic fibro-odontoma (AFO) — a benign growth composed of both soft tissue and developing dental hard tissues.
After careful surgical removal of the lesion and associated calcified structures, the specimen was submitted for pathology, confirming the diagnosis of ameloblastic fibro-odontoma.
✨ Key facts about AFO:
• Most commonly occurs in children and adolescents
• Often discovered when a tooth fails to erupt normally
• Typically appears as a well-defined radiolucent lesion containing tooth-like calcifications
• Surgical removal is usually curative with an excellent prognosis
Early evaluation of delayed tooth eruption can sometimes uncover conditions that would otherwise go unnoticed.

👅 Adult Frenectomy: It’s Not Just for Kids
Many people are surprised to learn that a restrictive tongue tie can persist into adulthood and may contribute to:
✔ Difficulty with tongue mobility
✔ Speech concerns
✔ Tension in the jaw, neck, or floor of the mouth
