Early detection of type 1 diabetes (T1D) gives patients and families time to process the diagnosis while gaining skills and knowledge needed for lifelong diabetes management. Unfortunately, T1D screening is still done primarily in research-based settings, and many frontline clinicians are underprepared to provide screening and support. This Perspectives article looks at current challenges in T1D screening and strategies for supporting the integration of T1D screening into routine health care encounters. https://bit.ly/4tMPtZN

A 55-day-old infant with growth faltering, reflux, and intermittent noisy breathing presents with a soft midline neck mass that appears only when he cries. Despite the striking exam finding, he remains well-appearing, afebrile, and without respiratory distress, and initial laboratory results are unremarkable. This fascinating case highlights the importance of careful observation and imaging in uncovering an uncommon diagnosis. Read the full case to see what ultrasonography revealed. https://bit.ly/4fu8sox

This study found that approximately one-third of US children and adolescents continue to use dietary supplements, which are often implicated in preventable adverse drug events and are a leading cause of pediatric morbidity and mortality from poisonings in the US. The most pronounced increase in use was in alternative medicines, specifically probiotics and fiber supplements. Read the full study to learn more about what dietary supplements US children are using now. https://bit.ly/4fCagfa

TORCH infections remain a critical cause of congenital disease, but the landscape of perinatal infections is evolving rapidly. Advances in diagnostic testing, changing epidemiology, and emerging pathogens like Zika mean clinicians need more than the traditional TORCH framework to recognize and manage these infections effectively. This month’s PIR offers a practical update on clinical presentation, interpretation of serologic testing, maternal-fetal transmission, and long-term outcomes with guidance that can directly impact patient care. If it’s been a while since you revisited congenital infections, this is an important read for anyone caring for newborns, infants, or pregnant patients. https://bit.ly/4tMHfkH

Pediatric vaccine uptake declined during the COVID-19 pandemic; however, little is known about the pandemic’s effect on hospital-based immunization. In this retrospective multicenter study, the Cerner Real-World Data electronic health record database was used to determine pediatric inpatient vaccine delivery rates for the prepandemic (May 2017 to April 2020) and postpandemic (May 2020 to April 2023) periods. Inpatient vaccine delivery generally increased over time; however, slopes of vaccine delivery decreased in the postpandemic period for IIVs and birth-dose Hep B. Further research is needed to understand these patterns and optimize hospital-based vaccine delivery. https://bit.ly/3PKkOOW

Traumatic hyphema—a common complication of penetrating or blunt eye injury—can lead to vision loss or long-term visual impairment, yet many pediatricians are not trained in the evaluation, treatment, and complications of these ophthalmologic emergencies. This article, complete with 14 images and figures, arms pediatricians with the tools for managing initial and follow-up care. Read the full article to be better prepared in your clinical practice. https://bit.ly/4nwXDnC

Youths and young adults with medical complexity perceive both disability-based discrimination and disability-affirming interactions with clinicians that impact their health and well-being. Understanding the lived experiences of youths in their own health care highlights important opportunities to improve disability training and promote inclusive care. This study explores the perspectives of youths and young adults with medical complexity and disability on their own care. https://bit.ly/3Rhh4oy

Each year, influenza leads to many pediatric illnesses, hospitalizations, and deaths. Vaccination remains the best way to help protect children 6 months and older from the flu, and a clearer understanding of vaccine protection can support more informed counseling for families. Using 2021–2024 data from 7 US pediatric medical centers, this study looked at how well influenza vaccines protected against both hospitalization and outpatient visits. Depending on the season, vaccine effectiveness ranged from 34% in 2021–2022 to 60% in 2023–2024. In both studies, protection was stronger against influenza B than against influenza A. Influenza vaccination is the best way to reduce the risk of influenza for children. Improved vaccine uptake would prevent additional influenza-associated hospitalizations and outpatient visits. https://bit.ly/4dwq3ux

Although many pediatric hospitalists work in community hospitals, little is known regarding fellowship preparation for community pediatric hospital medicine (CPHM) roles, including nonclinical competencies. This study aims to explore fellowship graduates’ perspectives on valuable knowledge, skills, and attitudes (KSA) needed in CPHM and to uncover educational gaps in pediatric hospital medicine (PHM) fellowship training. The authors' findings highlight important KSA necessary for CPHM roles, which are potential educational opportunities during PHM fellowship. https://bit.ly/4tMimFA

Food allergies are an important public health concern in the United States, and delays in giving epinephrine can make food-related anaphylaxis much more dangerous. In Utah, children faced added risk for anaphylaxis at school because many schools lacked nurses, staff training, and a consistent approach to food allergy emergencies. Working with the Utah School Nurses Association, the Utah Food Allergy Network, the Utah Department of Health & Human Services, and other partners, the authors set out to: (1) create and launch a web-based training program to help school staff recognize and respond to anaphylaxis; (2) inform school leaders about existing legal protections for layperson first responders; and (3) support schools in obtaining unassigned epinephrine through charitable pharmaceutical programs. https://bit.ly/4tou6ho
