Sometimes it looks small on paper: a ride to treatment, a food box, help with childcare. But when one barrier comes down, everything around it shifts, stress eases, options open up, and people have a little more room to breathe and plan. That “one thing” often spills over into health, work, and family in ways the system rarely measures.

At Jackson Care Hub, we start exactly where people are. If it’s one urgent need, we meet that need. If it’s a list of things, we walk that list with them. What matters most to us is what matters most to the person asking for help. Our job is to make sure they don’t have to navigate it alone.

Jackson Care Hub works hand in hand with the healthcare system. When patients get clinical care, they also get a social needs screening at least once a year. If they share a need and agree to help, that information goes to Jackson Care Hub. A Community Health Worker steps in, connects them to local services, and sends updates back to the clinic so the care team can see the full picture. Last year, we took time to update the Jackson Care Hub brand. We love our work and we want to share it far and wide!

The rebrand tells the true story of Jackson in a new way. It lifts up local stories of hope, grit, and shared responsibility. It shows a clear promise to build a community where everyone can thrive.

This new brand makes our quiet work visible. It shows how partners in Jackson use shared data and a common hub to respond faster when someone needs help. It reflects a system built on collaboration, dignity, and care for our neighbors.

Special thanks to all our community partners for sharing their stories with us. And a loving shoutout to our Creative Director Janeen Ritson and media partner . We are blessed to make good trouble with such magical folks!

Our partner agencies aren’t just “users” of the Jackson Care Hub, they are the heart of it. Every day, their teams show up for neighbors across Jackson with food, housing support, transportation, childcare, and more.

Our mission is to make their work easier. The Hub gives partners a simple way to connect, share referrals, and collaborate so our people get the right help faster. When our partners are able to work together smoothly, our whole community feels the difference.

In Jackson, health means more than a prescription pad. It starts with each person having a safe place to sleep, enough food in the fridge, and stability at home.

The Jackson Care Hub exists to hear and address those social needs alongside clinical needs, not after them. Because for someone who is unhoused or worried about the next meal, housing and basic stability are often the first treatment, long before medicine can do its job.

Clinical care does not happen in a vacuum. Housing, food, transportation, childcare, and other social needs show up in the exam room every day. If healthcare providers want better health outcomes, social needs have to be seen and addressed.

The Jackson Care Hub gives providers and partners a way to do both: identify social needs in real time and access a network of agencies who can help meet them. It’s one shared system that connects clinical care to the supports people need to actually get healthy and stay healthy.

In a single year, the Jackson Care Hub will complete about 250,000 screenings. These screenings don’t just connect people to support in real time, they also generate insights we use to spot service gaps and drive coordinated action across Jackson County.
