🔘once yearly for a full physical (to include cancer screenings, blood work, cardiac monitoring)
🔘once yearly for the Medicare Annual Wellness Visit
🔘every 4 months for management of chronic disease (to include blood work and medication monitoring)
🔘as often as needed for a sick visit
🔘once a month for weight loss education and management
