When a person passes away, it is the responsibility of the funeral home to generate and file the death certificate.

The deceased’s vital statistical information that must be obtained includes:

- Legal Name

- Birthdate

- Birthplace

- Address

- Social Security Number

- Marital Status

- Parents’ Names (including mother’s maiden name)

- Education Level

- Race/Ethnicity

- Occupation and Industry

Depending upon the state in which you live, additional information may be required. The death certificate will also document the disposition of the body (including burial or cremation), name of the cemetery, etc.
