Dear Patients:

It is with mixed emotions that I announce my retirement from active practice, effective
February 28, 2023. It has been a great pleasure providing for your health care needs over the years, and this has not been an easy decision.

Your medical records are confidential, and a copy can be released to you. You may make arrangements by calling the office at 812-634-6700 to coordinate a time to pick up your records through February 28, 2023. If you need your records beyond that time frame, you may still call 812-634-6700 or if you prefer, you can email [email protected] beginning March 15, 2023 to make arrangements to receive your records.

I have valued our relationship greatly. Thank you for your loyalty and friendship over the years.
