Huntsville: 2 RN Case Managers
Livingston: 1 RN Case Manager
💼Our RN Case Managers coordinate and manage the care of hospice patients, ensuring comfort, dignity, and support throughout their journey.
Responsibilities include:
- Managing a patient caseload
- Coordinating with the interdisciplinary team (IDG)
- Completing comprehensive assessments and care planning
- Providing education and support to patients and families
- Ensuring timely, compliant documentation
