Countryside Health & Rehab is proud to announce that we received a Deficiency-Free State Survey!

In the long-term care industry, achieving zero deficiencies is an incredible accomplishment and a testament to the dedication, compassion, and teamwork of every member of our staff. State surveys are extensive and evaluate every aspect of resident care, safety, and quality, making a deficiency-free survey a rare achievement.
We are grateful to our amazing employees for their hard work, to our residents for allowing us the privilege of caring for them, and to their families for trusting us each day.

This accomplishment reflects our ongoing commitment to providing the highest quality care and creating a place our residents are proud to call home. Congratulations, Team Countryside! 💙

🇺🇸❤️ Happy Independence Day from all of us at Countryside Health & Rehab! ❤️🇺🇸

We had an amazing Fourth of July celebration filled with laughter, great food, and unforgettable memories! A huge thank you to our wonderful dietary team for grilling up delicious hamburgers for everyone to enjoy. 🍔

The fun didn't stop there—we cooled off with a water balloon fight, and our residents had an absolute blast getting to dunk staff in the dunk tank! 💦😂 There were lots of smiles, laughter, and friendly competition all around.

Thank you to everyone who helped make today so special. Wishing all of our residents, families, and community a safe and happy Fourth of July! 🇺🇸🎆

Her grace, kindness, confidence, and beautiful spirit made her shine, and we couldn’t be more proud to have her represent our nursing home.

This is an exciting milestone not only for our new queen but for our entire Countryside family. She will now go on to represent us at the Regional Nursing Home Pageant, and we know she will make us proud every step of the way.

We are incredibly grateful to Legacy Hospice for donating delicious snacks and thoughtful t-shirts for our hardworking CNAs. We would also like to thank CenterWell Home Health for treating our staff to ice cream, which was a sweet surprise everyone enjoyed!
